從解剖學角度來看人腦，額葉與頂葉以中央溝分界。而外側溝則將額葉與顳葉一分為二。

額葉可以分為側部、額極部、眶部、和內部。每個部位由其各個腦回組成。

額葉在自主行為中產生重要作用。負責譬如行走等行為的運動皮質就位於額葉中。額葉的作用包括判斷當前行為的後果，各種行為之間的抉擇（更好、更壞、最好、最壞），控制、壓制對於社會來說不可接受的反應以及分辨事物之間同異。

額葉在整合大腦中非任務性長期記憶中扮演關鍵角色。這些記憶通常來自邊緣系統輸入而產生的情緒，額葉修葺這些情緒以便其與社會規範大致相符。測量額葉功能的心理學測試一般包括手指敲擊（目的是檢驗其自主行為）、威斯康辛卡牌分類測試以及語言、計數能力等。<ref>Kimberg DY, Farah MJ. 短暫性腦缺血發作和中風是65歲或以上人士額葉損傷的主要誘因。這兩項損傷可能是腦動脈中的動脈瘤破裂從而導致通往大腦的血流的阻塞造成的。其他造成損傷的因素包括外傷性腦損傷所導致的伴生事故，阿爾茨海默病、帕金森以及額葉癲癇。<ref name="Stuss">Stuss DT, Gow CA, Hetherington CR. 額葉遭受損傷的病人或許明白如何適當地對特定情境做出反應，但在實際生活中卻對同樣情境做出不當反應。個人所感受的情感可能無法通過面部和聲音表示出來。例如：開心時不會微笑、聲音聽起來不帶情感。但與此同時，病人有時又會展現出無根據的、過度的情感表達。中風患者中常有抑鬱症伴隨並影響了許多病人，隨抑鬱而來的則是生活積極性的減退和喪失，他們可能不想完成日常工作或者覺得「沒勁」。<ref name="Stuss" />與病人親近的人們可能會覺得病人的人格有所改變。<ref>Rowe AD, Bullock PR, Polkey CE, Morris RG.
