由上而下依序為分子層（molecular layer, I層）、外顆粒細胞層（External granular layer,II層）、外錐體細胞層（External Pyramidal Layer,III層）、內顆粒細胞層（Internal granular layer,IV層）、內錐體細胞層（Internal Pyramidal layer, V層）、多形細胞層（layer of polymorphous cells, VI層）。

高級動物的大腦皮質是包裹在大腦外側的連通皮狀結構。溝和回是皮質最為顯著的兩個解剖特徵。皮質有些區域向內凹陷，形成稱為「溝」的解剖結構。溝之外向外凸出的區域稱為「腦回」，越高等的動物腦回皺褶就越多，大腦皮質面積也越大。成年人類的皮質如果全部展平，面積大約有兩張報紙展開這麼大。大腦皮質的厚度約為2-4毫米。

皮質是由神經細胞組成，包括神經元和星形膠質細胞等其它支持細胞。大多數神經元屬於錐體細胞形態，其餘形態有籃狀細胞等。成年人類大腦皮質所含的神經元的數量大約在1010量級。皮質神經元之間形成大量的突觸連接。這些突觸連接包括分區內的連接、分區之間的側向連接和半球之間通過胼胝體的連接、以及和腦的其它部分（例如丘腦、基底核等）形成的連接。

在高級哺乳類動物中，大腦皮質存在分層結構。在新皮質，由外向內依次為1到6層。不同的層的細胞類型和分布，以及投射模式不同。

根據空間位置，大腦皮質被分為幾個腦葉。每個葉是空間上連通的一部分皮質。以下列出的是這些分區的名稱及目前學術界所認為的主要功能<ref name="#1"/>：

根據進化史，大腦皮質分為新皮質（Neocortex）和異源皮層(Allocortex)。異源皮層主要包括嗅覺皮質（olfactory cortex）和邊緣系統。

大腦皮質分為左右兩個半球。兩側半球在功能上有分化。在大多數人類中，語言、意念、邏輯、理性主要由左腦掌管，而右腦負責形象思維和情感等。例如：科學家比較理性、有邏輯，所以左腦發達；藝術家重感性、具創造力、擅長空間和物體形狀認知，故右腦發達（以上說法並不被當代神經科學、心理學界支持。多數功能實際上需要左右腦同時參與）。連接兩側半球之間的大量軸突形成一個稱為胼胝體的解剖結構。在身體下方相對應腦的主管區域越上方、越精密的動作器官佔的區域越大。
