選擇性緘默症（英語：Selective mutism）是一種社交焦慮症，情緒行為障礙，患者有正常説話的能力，但在特定情境下就是說不出口。《精神疾病診斷與統計手冊》把選擇性緘默症描述為兒童的罕見心理失調。患有這種症狀的兒童和成人能正常說話和理解語言，但在某些社交場合卻無法說話。他們其他的行為和學習能力都正常。但是症狀的程度與保持時間顯然與害羞相異，譬如一個小孩在學校裏完全靜默，在家中卻能自由說話，如有陌生人在場可能又會令小孩靜默，不過如果小孩在跟朋友語音聊天的話他可能不會那麼安靜。
語言治療師會在醫院或診所診治選擇性緘默症，而有時候選擇性緘默症是廣泛性發展障礙或情緒障礙的症狀。
選擇性緘默症常會誤診為自閉症或亞斯伯格症候群，尤其是當患者在心理學家面前表現得特別退縮的情況下，從而可能使患者接受錯誤的治療方法。
此症原先的英文名為elective mutism，凸顯出對此症的普遍誤解，甚至心理學家也不例外：誤以為患者自己決定在某些場合緘默，但實際上是他們的極度焦慮迫使他們緘默，即使想說話，也不能發出任何聲音。為了表示其非自願性，1994年，其英文名從帶有自願含義的舊名，改為中性的selective mutism。
此症的發生比例仍不清楚。由於公眾極大部份不知道此症，因此很多個案並未發現。按有紀錄的個案統計，發病率為1比1000。但在《美國兒童及青年精神病學院雜誌》（The Journal of the American Academy of Child and Adolescent Psychiatry）一份2002年的研究中，發生比例則高至7比1000。目前還沒有找到任何此症的病因，但有證據顯示有遺傳因素，女性比男性患者多（Steinhausen and Juzi, 1996）。

通常特徵有：
- 在某些期望說話的特定社交場合（例如在學校），持續無法說話，即使在其他場合能夠說話。
- 此種狀況影響學習或工作表現、人際溝通。
- 此種狀況已持續超過一個月（不僅限於上學的第一個月）。
- 不說話的原因不是因爲對社交場合中的語言使用不了解或不習慣。
- 此種狀況不適合歸為某些溝通障礙解釋（例如口吃），也不僅於廣泛性發展障礙、精神分裂或其他精神障礙的時候發生。
患者焦慮時通常有以下表現，可能會被誤認為無禮：
- 覺得難以保持眼神接觸。
- 常常不笑，表情空白。
- 舉止僵硬不自然。
- 對通常需要說話的場合，感到特別難以應付。例如學校點名、打招呼、道謝和道別。
- 比別人更易憂慮。
- 會以戴口罩來掩飾自己心裏的缺陷。
- 對噪音和人群更敏感。

而在積極的方面，很多患者呈現出：

- 對他人的想法和感受敏感。
- 有很好的集中力。
- 善於辨別是非，有正義感。

ISBN 978-0-86388-280-7.</ref>，故年輕時即應積極接受治療。否則，症況只會自我加劇，且患者會被視為不說話的人，這令他們更難開口。這時轉換環境（如轉校）到他人不知道其症狀的地方，或許能有幫助。而學校方面，亦應盡可能對學生採取調適措施，例如：容許學生在需要開口說話或歌唱的評估作免考安排。
在患者處於青少年時期時治療有可能會更困難。
強迫患者說話不會有效，且通常只會令焦慮更強烈，從而加深症狀。他人常常把這表現當作出於自願及控制性的，因為這時患者會斷絕一切溝通和肢體語言，而被視為無禮。
針對性的治療方法取決於患者本身，像年齡及其他因素。通常對較年幼的患者用刺激漸消法。
